- 心疾患による死亡の多くは病院外での突然死です12。
- 院外心停止例の救命率は改善傾向にあるものの依然数%と非常に低いのが実状です3456。
- 院外心停止例の救命には、市民による心肺蘇生の実施が効果的とされますが、3分の2の症例では、救急隊到着まで これが行われていません3789。
- AEDの使用が1分遅れる毎に救命率がおよそ1割低下しますが、心停止から119番通報までに平均3分もかかっています31011。
- AEDの設置が進み、一般市民がAEDを使用することで、救命率が4倍に高まるとされますが、心肺蘇生法に自信がないためにAEDの使用をちゅうちょする人も少なくありません121314。
- 人工呼吸を行う心肺蘇生法の複雑さ、口をつけることに対する抵抗感、講習会に長時間を要することなどが、心肺蘇生法の習得、実施を難しくしていると言われています151617。
- 心停止からの救命のためには、一刻も早く助けを求め【コール】、AEDを取り寄せるとともに、強くて速い胸骨圧迫を絶え間なく行うこと【プッシュ】が重要です1118。
- 胸骨圧迫のみの蘇生法でも、人工呼吸を行う心肺蘇生と同等あるいはそれ以上の効果があることが、日本をはじめ、いくつかの大規模臨床研究で報告されています1920212223。
- 胸骨圧迫のみの簡単な蘇生法であれば、救命に参加しようとする人が増えることが期待できます232425。
- 呼吸が止まってしまったことによる心停止など、なかには人工呼吸の実施が勧められる場合もありますが、目の前で突然倒れる心停止の大多数に、コール&プッシュが有効です。
日本循環器学会は、十分な効果を期待でき、協力も得やすい「胸骨圧迫とAEDの活用」に絞った蘇生法の一般市民への普及を促すことで、院外心停止例の救命率向上を目指します。
